One Life One Tree
On the occasion of World Environment Day, 5th of June 2019, Wellkin hospital has embarked on a new, ambitious project to preserve the environment.
With every baby both at Wellkin Hospital, parents of the newborn can get an endemic tree planted at the forest of Ferney La Vallée, free of cost.
Local endemic trees are disappearing at a very fast and alarming rate. Hence reforestation has become one of our ultimate goals and we hope that families will join forces with us as we try to create a safer and greener environment for our future generations.
Each tree planted at the forest of Ferney La Vallée will be tagged with the newborn’s name. A certificate will be issued to each parent upon planting of the tree.
This offer is open to babies born at Welkin Hospital from 5th June 2019 onwards. We welcome parents to read the private policy and consent form, and fill in the registration form. For more information, contact us on email@example.com.

PRIVACY STATEMENT AND CONSENT
Wellkin Hospital, owned and operated by C-Care (Mauritius) Ltd (BRN C07002054), having its registered office at 5th floor, Rue de L’Institut, Ebène, wants you to know that we respect the privacy of your personal records and we will do all we can to protect that privacy. We strive to always take reasonable precautions and security measures to protect your privacy. The information provided on this form is collected for the purpose of the One Life, One Tree project only. Under this project, for every baby born at Wellkin Hospital, an endemic tree will be planted at Ferney La Vallée, which is owned by La Vallée de Ferney Co Ltd (BRN C07073610), having its registered office at 5th floor, Rue de L’Institut, Ebène.
When we collect your personal data in this form, we are regulated as a controller of that data under the Data Protection Act 2017 (the “Act”) or, where applicable, the European General Data Protection Regulations (“GDPR”) (together the “Data Protection Laws”). The personal data collected will be communicated to our marketing team for purposes of administering this project and posting of certificates. Our marketing team will thereafter communicate the new-born child’s first name and date of birth to La Vallée de Ferney (LVDF) Conservation Trust, which in turn will be used to produce a tag. Each tag containing the first name and date of birth of the new born child will be displayed around each tree. LVDF Conservation Trust will share the above-mentioned personal data with a printing company for the production of the said tags and for the printing of certificates. When the tree has been tagged, certificates will be delivered by LVDF Conservation Trust to our marketing team who will be responsible for posting these certificates to you as parents or authorised legal guardians of the new-born child.
The personal data listed above will be retained at the marketing office of the Hospital for the purpose of the One Life, One Tree project only. The minimum necessary information provided on this form will be disclosed to third parties who are directly involved in this project on a strictly need-to-know basis for purposes of performing the project. The patient’s information will otherwise be kept strictly confidential and processed in accordance with the Data Protection Laws. The personal data communicated will be securely stored until your consent is withdrawn or until such time as may be prescribed by law. You, or the new-born child upon reaching the age of 16, may write to us to request access to, rectification, restriction or erasure of your personal data or personal data. In case of any withdrawal of your consent at any specific point in time, we shall proceed to the removal of the tag within a reasonable period of time.
